As a Federally Qualified Health Center (FQHC), we serve almost 40,000 individuals annually in Marin County. Our mission is to promote health and wellness through excellent, compassionate care for all.The Triage RN position provides nurse triage services and health advice for MCC patients over the phone at our West San Rafael Clinic. The RN assesses and documents patient questions or concerns and provides nursing advice using approved evidence-based triage protocols. The RN schedules follow-up appointments or refers to emergency services as needed. Triage RNs gain experience in triaging a variety of acute and chronic conditions for our diverse patient population.This RN Triage position's primary focus is triaging patients via telephone, with additional on-site support for the care team. This involves heavy use of EHR and instant messaging for coordination of patient care. This position may also require working weekends and evenings.

  • Bilingual English/Spanish skills are required.
  • 3+ years of direct patient care experience required; community health and/or triage experience preferred.
  • Part-time opportunity - 24 hours per week.All employees are required to be fully vaccinated for COVID-19, including required boosters as a condition of employment, subject to limited exemptions.Responsibilities
    • Provides excellent customer service in all phone and in-person interactions.
    • Provides culturally sensitive, patient-centered care to all MCC patients.
    • Provides nursing care within scope of license and clinic policy and procedures.
    • Educates patients on disease processes, such as diabetes and hypertension.
    • Uses protocol and clinical judgment to designate appropriate dispositions for patients calling in for medical advice.
    • Documents subjective and objective information and patient plans for all triages.
    • Assists providers and performs procedures accurately and thoroughly per protocol.
    • Administers injectables and monitors patients per protocol.
    • Conducts assessments and triages medical complaints by phone and in-person.
    • Collaborates with patients' primary care team as needed.
    • Reviews provider result folders for urgent lab and diagnostic results.
    • Responds to patient requests for lab results and medical questions.
    • Assists in developing and maintaining clinical workflows.
    • Participates in required meetings.Qualifications
      • Associate or BA/BS degree required.
      • Current California RN License in good standing.
      • Current CPR/BLS certification.
      • 3+ years of direct patient care experience required; community health and/or triage experience preferred.
      • Experience with EPIC Electronic Health Records preferred.Required Skills and Abilities
        • Bilingual in English/Spanish is required.
        • Strong enthusiasm and willingness to pitch in whenever needed.
        • Ability to make quick and accurate assessments.
        • Good organizational skills.
        • Excellent interpersonal communication.
        • Functions well in a high-stress environment.Physical Requirements and Working Conditions
          • Prolonged periods of typing and sitting at a computer.
